or, The Role of Mineral Hair Analysis in the Sale of Food Supplements

Patrick Holford has set up a charity. Not poorly, fluffy kittens or unwanted donkeys, but a charity dedicated to helping kids do better at schools with better 'nutrition'. The charity is called Food for the Brain. Being against such a venture would appear to be like being against sunshine or trying to ban Christmas, but I have some genuine concerns about the nature of this charity and will be writing more soon, but for now, I would like to concentrate on one rather strange aspect of it.

The charity sets out to 'promote awareness of the link between learning, behaviour, mental health and nutrition.' Great. But this is no Jamie-Oliver-Throw-Out-The-Turkey-Twizzlers-And-Eat-Seared-Carpaccio-of-Beef style campaign. No, this looks like ION for kids - ideas from Patrick Holford's Institute of Optimum Nutrition being sold to parents who quite rightly want to do the best for their children.

So, we see the usual ION themes - 'optimum' nutritional plans, 'optimum' health, food supplement regimes, questionable 'allergy and intolerance' testing and, what I want to cover today, hair mineral analysis.

The Food for the Brain web site discusses supplements for your kids in some detail and states that the charity uses hair mineral analysis as a diagnostic tool to see what supplements children may need in their school projects. For the schools, Patrick recommends Higher Nature's Dinochews supplements, an organisation that Patrick Holford, funnily enough, formulates products for and lends his name to. There is also a link to a site called MineralCheck that appears to give independent advice about minerals and hair analysis. (More on this later!)

The idea that our diets may be deficient in minerals, even if we are eating a balanced diet, is popular in nutritionist circles. There are ideas out there in nutri-land that our soils may be depleted from nasty intensive farming and that we should be taking the right supplements to top up. However, scientific sources for this are hard to come by and invariably appear to originate from the suppliers of mineral supplements, as I have previously discussed.

Now, as an idea, diagnosing exposures to heavy metals and attempting to diagnose nutritional mineral deficiencies through analysing hair samples, has been around for some time. The only problem is that it looks like it does not work and has been shown to be flaky in a number of reviews.

Stephen Barret, one of the first reviewers to look at the subject concludes,

Hair analysis is worthless for assessing the body's nutritional status or serving as a basis for dietary or supplement recommendations. Should you encounter a practitioner who claims otherwise, run for the nearest exit!
Why would a charity, wanting to improve the nutritional status of kids, recommend to put them through doubtful diagnostic techniques? Before we come to any conclusions, let's look at why hair mineral analysis probably does not work. It might involve a little science. Forgive me, I think it is worth it.

So, according to MineralCheck, Hair Mineral Analysis (HMA) claims to be able to determine if you have an 'imbalance' in minerals. You get a report back telling you about:

  • Your body's level of nutrient minerals and toxic metals Mineral ratios
  • A list of recommended foods - and those to avoid
  • Food allergy indicators
  • Body chemistry balance analysis Suplement [sic] recommendations

Email the company and they tell you the following...

The cost of the test is £49 and the laboratory will test for 29 nutritional minerals including calcium, copper, zinc, sodium, potassium, magnesium etc) and 8 toxic minerals (including lead, aluminium, mercury and cadmium). The results are presented as a graph with a report attached explaining them and making diet and where appropriate supplement recommendations. Your sample can be sent by post and the report is returned by post.
All very impressive from a few strands of hair. The problem is that these sorts of analytical techniques are very hard. You are trying to find the levels of trace amounts of large numbers of metals in biological samples and then relate that analysis to an understanding of human physiological function and health. This is the stuff that a hundred PhDs are made off. Careers are devoted to such techniques. Let us walk through some of the questions that would have to be well answered by sound science if we are to get close to the MineralCheck promise...

  1. How does the mineral concentration in hair relate to whole body concentration?
    We are not actually interested in hair concentrations as such, but the levels in tissues that need the minerals, such as the blood and other organs. Does hair take up minerals in direct relation to body concentration? It need not. We need to know the answer to this question for each element being analysed.
  2. What individual variation is there in hair growth and mineral levels?
    How does age, ethnicity, sex, activity levels and health affect the result? Again, we need to understand this for each element being tested.
  3. What levels in hair are normal and what ranges are acceptable?
    And how does this vary across different geographies with different diets and lifestyles?
  4. How can we relate these levels to health issues?
    Even if a mineral level is outside the normal range, this does not mean that there is a problem. Mineral levels may be biologically unimportant within a wide range.
  5. How best should we collect samples?
    Does using steel scissors introduce contaminants? What about any sample packaging used? Is 'home' sampling OK, or do you need controlled lab conditions?
  6. How much hair do we need to get a reliable, repeatable result?
    One strand, a bunch, how long should the hair be?
  7. Do we need to prepare the sample to remove environmental contaminants?
    Shampoos, car fumes, cigarette smoke and general dirt will be on the hair. Can this be easily removed? Is it absorbed into the body of the hair? Do hair treatments, such as bleaching and colouring, affect the result and how?
  8. What analytical technique is best?
    Most techniques are poor at measuring wide ranges of elements, but are good at targeted elements. Do we need several techniques or the same technique optimised in many ways?
  9. How do we ensure the right levels of accuracy and precision at an affordable level to the testing laboratory and their customers?
    It is no good having a whizzy technique if it costs millions.
  10. How do we get good calibration samples?
    In order to get good results, you need good standard samples to compare against. How can a laboratory obtain known and certified reference materials for each mineral being tested at concentrations similar to that being tested? What analytical technique should be used to certify the references?
  11. How should Hair Mineral Analysis laboratories undertake external quality assurance?
    Good laboratories validate themselves against other independent laboratories to make sure they are not systematically getting this wrong. Who will do this?
  12. What do bald people do?

Now, the problem is that there are few answers to these questions and much work to be done. It could be one day that we answer these question in sufficient detail to have hair analysis as a useful diagnostic tool. But we are not there yet, and one of the reasons is that other more direct techniques, such as blood or urine analysis, are better tools to put our research energy into.

Given the poor state of the science of hair mineral analysis, one might expect that laboratories offering this service might lead to shabby, inconsistent and meaningless results. And that is what is found. Several studies have looked into the quality of results obtained from commercial laboratories. One 1985 study entitled, "Commercial hair analysis. Science or scam?" concluded,

The reported levels of most minerals varied considerably between identical samples sent to the same laboratory and from laboratory to laboratory. The laboratories also disagreed about what was "normal" or "usual" for many of the minerals. Most reports contained computerized interpretations that were voluminous, bizarre, and potentially frightening to patients.

Six laboratories recommended food supplements, but the types and amounts varied widely from report to report and from laboratory to laboratory. Literature from most of the laboratories suggested that their reports were useful in managing a wide variety of diseases and supposed nutrient imbalances. However, commercial use of hair analysis in this manner is unscientific, economically wasteful, and probably illegal.

As this was 20 years ago, we ought to be cautious, but luckily a similar study has been done more recently to see if things have improved. It concluded,

...Variations also were found in laboratory sample preparation methods and calibration standards. Laboratory designations of normal reference ranges varied greatly, resulting in conflicting classifications (high, normal, or low) of nearly all analyzed minerals. Laboratories also provided conflicting dietary and nutritional supplement recommendations based on their results.

CONCLUSIONS: Hair mineral analysis from these laboratories was unreliable, and we recommend that health care practitioners refrain from using such analyses to assess individual nutritional status or suspected environmental exposures. Problems with the regulation and certification of these laboratories also should be addressed.

It is no surprise then that QuackWatch calls this technique a 'Cardinal Sign of Quackery'. Even worse, the American Medical Association issues a policy on the technique which states,

The AMA opposes chemical analysis of the hair as a determinant of the need for medical therapy and supports informing the American public and appropriate governmental agencies of this unproven practice and its potential for health care fraud.

So, why do people including MineralCheck continue to carry out such analyses? It is difficult to conclude anything other than it is very lucrative and a good way of pursuading people to buy supplements. Whilst blood analysis needs qualified practitioners to take and analyse the sample under medical conditions within a strict legal, ethical, and scientific framework, hair analysis requires none of this. Its much easier, and importantly, much cheaper; posting off a hair sample and getting a computer read-out back. Follow that up with recommendations to buy £50 worth of supplements per month, an order form, and a recommendation to repeat the test in a few months time and you are quids in.

Worryingly, there is a danger that, as the technique looks near useless as a diagnostic tool and the recommendations that come from it arbitrary, there is not only the risk that customers will waste their money, but that harm may come too from needless and drastic changes in diet and excessive supplements.

And, as promised, what do we know about the background of the web site MineralCheck? They don't say much on their pages - no names, no company information, an anonymous email address, but they do give a telephone number. A quick Google reveals that this telephone number is also used by a Mrs Karen Watkins BA(Hons), Dip.I.O.N, MTTS. It turns out that as well as doing Hair Mineral Analysis, Karen is also Principle of Education at Patrick Holford's Institute of Optimum Nutrition.

My advice for any school or parent involved with programmes to improve kids nutrition, and is using Food For the Brain for help, should be to question Patrick, the Charity Trustees and the Scientific Advisers to the Charity very hard about the value that Hair Mineral Analysis is bringing to the children. If you get evasive answers, particularly questioning the qualifications of those who doubt the advice from Food for the Brain, I suggest you follow Stephen Barret's advice and 'run for the nearest exit!'

The First Annual Quackometer Awards and Year Review

The Quackometer has been up and running for just about a year and has seen some serious traffic increases over the past six months. Starting off with just a few hits per day, the daily unique visits now stand in the many hundreds, with a peak recently of several thousand. Cripes! What started off as a bit of a bored joke has now grown into a proper web site.

So, a quick, tearful thanks to all the people who keep coming back. Thanks for all the correspondence, both encouraging and threatening. I hope 2007 will throw up richer, funnier and more useful functionality and content. Plus a few more innevitable threats.

So, to the main business. The quackometer scans various news sites twice a day on the look out for quack stories. I thought is would be good to review what has been found, where the stories are coming from and who is writing them. Awards will be made.

But first, an apology and admission. This is going to be very UK-centric - my time has very much concentrated on the UK press at the expense of many sources of potential quackery around the world. Maybe, I can get this working better next year for overseas news sources. I must say though, that the UK does look like its newspapers are particularly prone to printing quack nonsense. More research is needed to see if this true. Also, I must point out that the Quack News Scanner was only working from August - so not a full year yet in review.

And now for he disclaimers. This is not a scientific study! I make no bold claims to have conducted a comprehensive review of all the papers and I have not done extensive validity checking on all the spotted articles. Life is short and it is just for fun. Also, not all papers are represented. The Independent and Express are the big omissions (for technical reasons) and this is a shame since the indie spurred me on with a silly piece about electrosmog earlier in the year. Nor do I include the red-tops (bar the Mirror) partially for technical reasons, but mainly because they are different sort of beast where their readers engage with the paper in different ways than other more self-important titles. (My feeling is that papers like the Sun are not quite so credulous as one might naively suppose - I will be looking into this further). Finally, all the stories listed below, may not be quackery. As always, read and research and make up your own mind.

So, straight into the first award...

Quackiest News Source

The summary of scores for stories since the beginning of August 2006 is...

1) The Daily Mail with 38 stories and a total of 157 Canards
2) The Times with 30 stories and a total of 132 Canards
3) The Guardian with 15 stories and a total of 67 Canards
4) BBC with 8 stories and a total of 29 Canards
5) The Mirror with 6 stories and a total of 21 Canards
6) The Telegraph with 1 stories and a total of 3 Canards

(All papers include their Sunday equivalents)

So, hardly a surprise that the Mail (and Mail on Sunday) lead with 38 stories that scored over 3 Canards. The Times is not far behind. However, analysis of the data reveals a few interesting points. The Times score predominantly comes from its "Health alternatives" column. This is clearly flagging the stories as being 'alternative', or as we like to say here, 'not real'. The Mail on the other hand makes no such gesture to alerting its readers that bollocks may follow.

It is interesting to note, that the Guardian has the highest Canards per story ratio. Maybe this is because the Mail tends to let a bit of quackery slip into lots of stories rather than just concentrate on the big quack scoop. The Mirror's stories can be pretty much put down to one columnist, a Ms Gillian McKeith. No more to say there then - she has aggressive lawyers. And congratulations to the telegraph for only scoring 3 Canards for one story promoting osteopathy - but at least in an area where this technique has a chance of working.

So the winner of Quackiest News Source really has to be - The Daily Mail - Congratulations!!

A well deserved win. Its continuous commitment to publish rubbish health stories coupled with very few warnings to its readers that what is going to follow is complete nonsense mean that it was hard to beat this year. Furthermore, its commitment to give telephone numbers and web addresses of quack suppliers will undoubtedly result in many of its moderately wealthy, middle-class readers handing over their hard-earned dosh to the fraudulent and deluded. Despite the Mail's aversion to tax of all forms, this is undoubtedly the Mail's facilitated tax on the gullible.

Quackiest News Story

At the end of this blog, I have given a list of all stories the quackometer found that scored over 5 Canards.

A couple of smashing stories really stand out. Dr Danny Penman's remarkable story about the healing properties of prayer was quite special. Also, the Times mindless plug for that rather silly technique Bi-Aura stood out from the crowd. But, by a country mile, the most ridiculous and credulous story of the last four months has to go to Sarah Stacey for that outstanding piece of work Good vibrations in the Daily Mail. The Quackometer spotted it, gave it 10 Canards, and it is difficult to niggle with that analysis.

The story plugs several different 'therapies' - all for a made-up illness and, at least in the case of the QLink pendant, it is difficult to conclude anything other than it is fraudulent. The QLink is a classic piece of pseudoscience, invoking quantum theory to explain its non-existent properties. There is a cast of thousands in the story, all offering testimonials for the QLink trinket, including Dr Wendy Denning (who still cannot spell complementary), Professor Jobst and Dr Mark Atkinson. Oh, how I love titles.

Quack Journalist of the Year

for her unwavering commitment to writing and promoting all manner of quackery in the Health Notes section of the You supplement of the the Mail on Sunday. She has written a string on quacktastic stories, always with a good plug for the source, most often, Victoria Health. (If you join the VH Club, you can get a free Sarah Stacey book!)

So, the Mail has done remarkably well this year. Any surprise? Not really. As was recently well put on the badscience blog, nutritionism (or nutriquackery) is a particularly right-wing pastime with an obsession for personal responsibility for your health rather than looking to the wider society for causes and solutions. Thus, it is only your own fault if you are fat and poor, unhealthy or have badly behaved and underachieving kids. Pop a supplement pill to improve kids GSCE results rather than support and send your kids to the local school. The Mail's whole point of view is based around a distrust of any authority that could challenge its small minded world view. Science and scepticism are direct challenges to the myths and delusions of its approaches to the problems of health, government, immigration and economics. No wonder quackery thrives.

Quack Word #16: 'Nutritionist'

A regular comment to me is to ask "why have I got it in for Nutritionists?" Surely, these are dedicated health professionals who do wonders for peoples' health by improving their diets and making sure people take the right supplements, if required. Well maybe. The problem is that so many nutritionists are not doing this and often resort to pseudoscience and quackery. This week's Quack Word blog entry will argue that the Quackometer is quite right (most of the time) in scoring highly a web page with the word 'nutritionist' in it.

So, a quick definition of 'nutritionist'. Whilst one should always take wikipedea articles with a sceptical eye, their definition of nutritionist is a good starting point:

A nutritionist is a person who advises people on dietary matters relating to health, well-being and optimal nutrition. Nutritionists should not be confused with dietitians. Dietitians are health care professionals who have received specialised formal accredited tertiary education and training, and undertake internship in hospitals, and who are required to adhere to their regulatory body's code of conduct. They are also the only non-medically-trained health-care professionals permitted to practise clinically in hospitals or health-care facilities. Many "nutritionists" appear on television, in newspapers and magazines, and write bestselling nutritional books.

So, there is our first major cause for concern, anyone can call themselves a nutritionist. Call your self a dietitian without a formally recognised qualification and you would be breaking the law. But, if you just want to write in a Sunday supplement or set up a health food web site selling vitamins, by all means, call yourself a nutritionist.

The wikipedia goes on:
Self-identified nutritionists have varying levels of education, and can be someone with little education up to someone who may have the equivalent of a master's degree in Physiology or Biology.
I have discussed before how some high profile UK nutritionists have little formal education, like to flaunt their unconventionally acquired titles and awards, and glow under self-styled accolades, such as 'world's foremost nutritionist'.
Now, there are varying trade associations that do seek to represent nutritionists in the UK. Membership is not compulsory and of course, they cannot stop someone calling themselves a nutritionist if they act in a way thought to be harmful or dishonest. Some appear to have little interest either in monitoring the behaviour of their membership as was well documented by Ben Goldacre of the Guardian when investigating The Nutrition Society.

But, surely this is all a side issue - getting people to eat healthily is what counts? Well yes, but I will argue that the advice of so many of the Sunday supplement writers can actually be counterproductive. Let me list some ways in which nutritionists go astray...
  • It's not just about eating healthily. Bad diet is promoted as being the root cause of almost all diseases and conditions. Eating in a certain way can restore the 'balance'.

  • It is not possible to get all your vitamins and minerals from food today because of modern farming methods. The nutriquack can sell you the right supplements.

  • Organic is healthier.

  • Claiming that a simple change of diet or popping a vitamin cure complex social issues, like omega-3 fish oil pills helping poorly performing kids in schools,

  • Promoting radical diets which usually involve cutting out entire food groups.

  • Promoting the health benefits of consuming huge volumes of vitamins.

  • Advocating 'superfoods' that allegedly have remarkable health benefits.

  • Obsessions with discredited and weird diagnostic techniques, such as examining stools.

  • They use pseudoscience to sound knowledgeable. Talk of 'detoxification' is common.

  • Selling weird made up foods with remarkable properties such as this nonsense salt seller and shrouding it in ridiculous claims.

All these things have in common is their overstatements and lack of evidence. Making health claims in this way is quackery. From now on, I will call such people the nutriquacks.

I think the problem of the nutriquack arises from the simple fact that good nutritional advice (for most people) is quite simple - eat a balanced, varied diet with a low amount of fat and lots of green stuff. You are not going to make a fortune with that mantra - even though getting people to follow it is quite hard sometimes. By making the whole thing appear more complicated though, the nutriquack is creating a market for their services. You cannot get enough antioxidants - my superfood berries (available on my website) will do it for you though! Register with my site, complete my questionnaire and I will personally compose your optimum nutrition plan and supplement mix. And so on.

What is happening is that nutriquacks are fetishising food and bamboozling people. Rather than enjoying food for its own sake, many people are led down the path of analysing everything they put in their mouth, jumping to conclusions about why they might be overweight or unwell and fruitlessly giving money away to people who do not deserve it. The real heroes of healthy eating for me are those people who try to instill a love of good food into people. Chefs and writers who try to excite about the benefits of buying good ingredients, how to source fresh ingredients inexpensively, how to be creative in the kitchen without needing top-chef skills and basically try to impart a joy about food. That is surely the route to people having a good, healthy relationship with their food and so end up getting a more rounded, varied and balanced diet. People like Jamie Oliver, Nigel Slater and Nigella Lawson spring to mind, but there are many more. These people do not resort to pseudoscience in order to justify what they do.

When science does make some well researched discoveries about the food we eat, this is often drowned out in the swamp of nutriquack baloney. It is often impossible to tell good science from nonsense in the popular press and TV. All this does is make people despair of the 'scientists' with their constantly contradictory advice and silly discoveries. It undermines a reliable source of knowledge for society that genuinely could help improve peoples' lives.

Nutriquacks operate in a legal void. Selling food is not illegal after all and vitamins and minerals are just food. However, make medical claims and use ingredients that might be medicinal in nature and you might end up in hot water. At least this is a curb on the excesses of nutriquacks, although it is seldom invoked.

However, such is the fate of arch-nutriquack 'Dr' Gillian McKeith. Today, the MHRA (the British organisation that is supposed to control the use of medicines) has ordered that McKeith stop selling illegal products. McKeith has been capitalising on her TV fame by selling all sorts of expensive and silly 'superfoods' to her fans. At last, the law has caught up with her, at least in a little way and she will have to re-think how she goes about her business now.

The Mineral-Depleted Food Scandal

The news (1, 2, 3, 4, 5) has been full of reports about how our food in Britain is becoming less nutritious and that it is becoming increasingly difficult to obtain a full set of minerals and vitamins through the food we buy in supermarkets. I have been told by several people that this is the reason why it is so important to take supplements. Can this be true? If so then it is truly shocking! We can no longer feed ourselves! What is going on here?

I've always thought that the best way to get your minerals and vitamins was to eat a varied and balanced diet, rather than take dietary supplements. I further believe that the major source of information about the need to take supplements comes only from the big business interests behind the 'health fraud' industry.

So I did some digging. The common source behind these allegations of Mineral Depleted Food appears to be a report issued by the Food Commission. Let's take a look at this.

First, the Food Commission is a consumer lobby group involved in doing consumer surveys and publishing 'thought leadership'. It does not in itself publish peer-reviewed research. Like all such groups, it has an agenda, and one that I probably broadly agree with. Nonetheless, given this, one should always be cautious in examining its claims.

The report in itself suggests that several foods (meat and dairy) have lower levels of minerals than the same foods had in the 1920s. There are several problems with this analysis:

  • If this is indeed true, there is nothing in this report to suggest that the levels have fallen to dangerous levels. Is it still likely that a varied and balanced diet will supply most peoples' needs? The report declines to comment.
  • There have been several criticisms of the methodology of this research (comparing government tables, the best part of a century apart). Analytical methods have changes enormously over this time and there is no correction for biases that will have been introduced as a result. (Remember the famous iron-in-spinach myth?)
  • The report goes on to show that 8% of women might be mineral deficient but (this is the important bit) the report does not say that this is a result of the (alleged) lower mineral levels in food. This could be down to these women just having very poor diets. This is to be expected, as we know some people do not eat well and there is no attempt to correct for this. This is either a little disingenuous or just plain not rigorous enough. The report allows the connection to be made in the readers mind - but does not state the connection itself.
So, not very convincing then. So what else does the Food Commission say on this subject then? Interestingly, it also publishes a report on vitamin and mineral fortification in diets (as promoted by many major brands, e.g. Nestle) and goes on to widely condemn the practice. One of the key findings in this report is that such practices are a subject of concern since it "promote[es] the concept of added nutrients as improving health, versus promotion of an overall healthy diet." In other words, the Food Commission says that you should eat a healthy diet; don't rely on added supplements.

It would appear to be case closed, but the story gets a lot better.

Why did the Food Commission publish this report? Who did the original research?

It is stated that the research was done by a Dr David Thomas. Now Dr Thomas was originally a geologist (alarm bells) and has "retrained as a chiropractor and nutritionist" (very loud sirens). Dr Thomas does not work at any academic institution doing research, as you might have thought given the seriousness of this report, but rather has been running a company that sells (drum roll) mineral supplements.

So, could it be that this report was originally just a piece of puff marketing released by a company that would directly profit from people believing it? I don't know. If it is just marketing then it is a scandal. Obviously many people are worried about their health enough to invest lots of money in unnecessary supplements.

Personally, I think the next time you are tempted to blow twenty quid in a health food shop on unnecessary supplements, you should keep on walking down the high street until you find an Oxfam or Save the Children collector and pop that twenty quid in their collection tin. The added nutritional value that the money will provide to struggling farmers in truly undernourished parts of the world will greatly outweigh any marginal benefit those pills will bring you – IF you eat a varied, balanced diet.
